Consent to the processing of personal data

This consent is granted pursuant to Act No. 110/2019 Coll., On processing of personal data, as amended (hereinafter referred to as “OCT”) with effect from 24 April 2019, and further with effect from 25 May 2018 pursuant to Article 6 paragraph 1 (a) | (a) Regulation (EU) 2016/679 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (hereinafter referred to as the 'GDPR Regulation').

I declare that the personal information provided is true. I give this consent free of charge without any compensation or reimbursement. I understand that the provision of personal data is voluntary. Furthermore, I acknowledge that I have the right to withdraw my consent at any time in writing, and the withdrawal of my consent is without prejudice to the lawfulness of the processing based on the consent given prior to its withdrawal.
